Interuniversity Athletics – Student Athletes
Wilfrid Laurier University is committed to protecting your health and safety while participating in Interuniversity Athletic Activities (the “Athletic Activities”) as a student athlete. Interuniversity Athletic Activities includes participation by a registered student athlete, coach, or athletic staff in any individual or team training, game, practice, meeting or engagement event. However, the University cannot protect you against all risks, particularly given the spread of the novel coronavirus and the emerging variants of COVID-19 and the nature of the interaction between individuals when participating in the Athletic Activities. This informed consent is intended to make sure you understand the risk associated with participating in the Athletic Activities given the risks associated with COVID-19 (including all known and emergent variants).
Wilfrid Laurier University has put in place measures to reduce the spread of COVID-19, however the University cannot guarantee that any individual attending the University Campus, using the University’s facilities, or participating in Athletic Activities organized by the University, whether on-campus or off-campus will not become infected with COVID-19. Further, attending the University Campus and participating in the Athletic Activities, could increase the risk of contracting COVID-19.
All participants in the Athletic Activities are being asked to carefully review, confirm, and agree to the statements made below. Please note: as public health and government guidelines may change, University plans and requirements may need to be revised. If so, participants will be advised accordingly.
Please review all statements.
- I understand that COVID-19 is extremely contagious and can spread by person-to-person contact.
- I understand that by participating in the Athletic Activities I may be exposed to or become infected with COVID-19, which could result in the following: extended quarantine/self-isolation, additional tests, hospitalization that may require medical therapy/intervention, intensive care treatment, possible need for intubation/ventilator support, short-term or long-term intubation, other potential complications (including lung and heart complications) and the risk of death.
- I understand that COVID-19 might cause additional risks, some of which may not currently be known at this time.
- I understand these risks could cause me to incur medical and other expenses that will not be covered by the University.
- I understand and will follow all applicable legislation and Public Health measures and guidelines. In addition, I understand that it is my responsibility to ensure I learn and follow all health, safety and other rules or policies and procedures implemented by the University or directions by Athletics & Recreation staff which may be amended from time to time.
- I understand that I must not attend the University or participate in the Athletic Activities if I or any one in my household is sick or symptomatic in accordance with all applicable legislation and Public Health measures and guidelines and will notify the Department of Athletics & Recreation as soon as reasonably possible. I understand that if I present symptoms I will be directed to be tested and follow all necessary processes as outlined by Public Health and the University and will notify the Department of Athletics & Recreation as soon as possible the results of a test. The Department of Athletics & Recreation in consultation with University staff will determine when I may be permitted to return to the Athletic Activities as a student athlete with the University.
- I understand that any behaviour on my part that places others at risk could result in immediate termination of my contract as a volunteer and part-time coach and my right/privileges to use the University’s facilities or participate as a student athlete with Athletics’ & Recreation.
- I have informed myself of the risks of COVID-19. I knowingly, willingly, and voluntarily consent to participate in the Athletic Activities in my role as a student athlete during the COVID-19 pandemic.
- I understand that I will be required to complete the daily COVID-19 Self-Assessment through the Laurier SAFEHawk App prior to attending any Laurier campus location or off campus events. If the self-assessment is not a “pass” indicating I present a risk, I will not attend campus and will follow all applicable Public Health requirements and review the Laurier Symptom and Close Contact Reporting for COVID-19 information for more information about reporting and case management. I acknowledge that Laurier and the Department of Athletics may institute a COVID-19 Testing Program as required by the University, Public Health, or a governing body and or. Testing may be required in order to return to your duties following an exposure to COVID-19 and/or fulfilling your role on behalf of the University.
- I understand that, the University has made vaccination for COVID-19 mandatory for student athletes and trainers to minimize the health and safety risks associated with participation in the Athletic Activities, and that vaccination does not fully eliminate the risk of becoming infected with or transmitting COVID-19 and agree to adhere to all the requirements set out in this COVID-19 Informed Consent.
- I understand that I will be required to complete and submit the Vaccine Attestation form located in my medical assessment form confirming my vaccination status by [the start of my training camp and must be fully vaccinated in order to travel and compete as a student athlete. The deadline to be fully vaccinated is September 30th, 2021]. If I am unable to be vaccinated due to medical reasons or for creed/religious beliefs protected under the Ontario Human Rights Code, I may submit the applicable request for accommodation by connecting with the Athletics & Recreation Department
I acknowledge that this Informed Consent is valid from the date noted below until the end of my contract as a student athlete and governs all the Athletic Activities in which I participate.
